Middle School Dance – Guest Permission Slip

All students attending the Academy of the Sacred Heart’s Middle School Dance are expected to:

• Respect all adults, chaperones, and school staff.
• Respect school property and the event space.
• Leave cell phones and electronic devices at home or check them at the door.
• Clean up after themselves and dispose of trash properly.
• Use appropriate language at all times.
• Demonstrate appropriate behavior and respectful conduct.
• Follow all school rules throughout the event.

Any behavioral issues may result in dismissal from the event and a parent/guardian being contacted for immediate pickup.
